Adfly hijacked my browsers and still there after formatting and installing fresh windows

Hi guys,
i have adfly redirecting any site i visit and i performed all things mentioned and adfly still there then i installed fresh new windows i opend IE and adfly still there !!! and redirected the msn home page of IE
and i have "authentication required" pop up randomly asking me to enter username and password
could you help me please
